Written Request for Leave

I feel uncomfortable all over today. In the morning, I felt sick to my stomach and vomited. After lying down all day, I woke up weak, had a headache, and had a little fever.

I don't know what the specific problem is, but it's very uncomfortable anyway. I can't sit still in front of the computer for a long time, and my eyes hurt when I look at the screen, so I have to decide to take a day off.
